You agree to provide accurate information, attend sessions on time, and use the service in a respectful and lawful way. The practice may reschedule or decline sessions where safety, capacity, or suitability requires it.
By booking, you acknowledge that electronic communication, messaging platforms, and online forms have privacy limitations and are not the same as in-person confidentiality.
Consent and records
Clients must confirm that they have read the privacy policy and terms before booking. Signature capture may be stored with intake and booking records as evidence of consent.
Intake documents, booking records, and consent records should be retained only for as long as needed for the practice and for any lawful record-keeping obligations.
